I am developing a mobile application with Xamarin but I am still in doubt whether I should add a project for iOS, the goal is for a very small group of people who will use my application, you can distribute this application without publishing it in the App Apple Store?

Yes you can, but you need to be in the Enterprise Program which is a membership that is paid annually of US $ 299.

You can try TestFlight in closed beta mode, which allows you to distribute it to only a small group of people using the Apple ID. Users need to install the TestFlight app from the App Store which works as a sort of second App Store (worth the redundancy), allowing you to receive notifications when you are invited to use the app or have an update.
